Once you submit the form, we will contact you by email or phone to confirm your booking and provide you with a time slot.

After your time slot is booked, you will receive a confirmation email. If you need to cancel or reschedule, please do so at least 24 hours before your appointment.

One of our CSWs will then contact you and help you complete all the information needed for the Extra Care Register. Any support provided through this service is completely free.
